fix: preserve release changelog history#13

Release preparation was overwriting CHANGELOG.md with only the next generated section. That made previous release notes easy to lose and left pull request references as plain issue-shaped text. Route changelog generation through git-cliff --prepend, restore the missing release entries from tag history, link PR references, and document upgrade plus commit-message expectations for future public contract changes. Verification: bundle exec ruby -Itest test/release_task_test.rb; bundle exec standardrb Rakefile test/release_task_test.rb; git-cliff -c cliff.toml -o /tmp/indexmap-changelog-smoke.md; git diff --check.
